Fill-in the downloaded form and affix your signature.

Attach documentary requirements.

Visit the Business Bureau Office and submit the filled-in and signed form together with the supporting documents.
Submissions are accepted from Monday to Friday, 8:00am to 5:00pm.
Admission to Business Bureau premises is limited to exercise the required health standards for the existing quarantine procedures.
The Business Bureau Office is located at Ground Floor, Sangguniang Panlungsod Bldg., San Pedro St., Davao City.

The Business Bureau personnel will review and validate the form and documents submitted. Upon approval, you are now ready to do ONLINE ORDER OF PAYMENT.
